Once you have downloaded the form, please complete all highlighted areas. You can fax your form to the Medical Records Department at 843-784-8006 or bring the form to Coastal Carolina Hospital, 1000 Medical Center Drive, Hardeeville, SC 29927.

There may be a fee to copy records. Once you have paid the agreed upon fee, your request will be processed and available for pick up between 8 a.m. and noon, Monday through Friday; or at an alternate time if these hours are inconvenient. Please allow 2 to 5 business days for processing.
